Silver City, NM (February 4, 2019) Hidalgo Medical Services (HMS) received a multi-million, multi-year grant from the Substance Abuse and Mental Health Services Administration (SAHMSA). HMS, in collaboration with the New Mexico Human Services Departments and the University of New Mexico, has launched a program called “Bridges 2 Wellness” that assists in improving care coordination and patient access to care. This program significantly increases collaboration between behavioral health and primary care providers through increasing health promotion, coordination of care and integration services. These services are focused on improving the overall health and wellness of adults with mental illness and substance use disorders who also have chronic physical health conditions.   

The “Bridges 2 Wellness” program is one of two new programs introduced recently by HMS. The “HMS CareLink NM” program is funded by the State of New Mexico Human Services Department and whose aim is to enhance integration and coordination of primary, acute, behavioral health, and long-term care services and support for persons with chronic medical and behavioral health conditions across their lifespan.  Between the two programs, most patients will be able to access this collaborative system of health care led by HMS with the aim of improving quality of life for individuals with mental illness and/or emotional disturbance.

In a statement by Dan Otero, HMS Chief Executive Officer, “There are only two healthcare organizations across the state of New Mexico, HMS and another entity in Lea County, that were selected for the ‘Bridges 2 Wellness’ health initiative. HMS is very pleased to be a part of this person-centered system of care that addresses an individual’s holistic health and wellness, that will help serve the residents of southwestern New Mexico.”

Specifically the “Bridges 2 Wellness” eligibility requirements include being 18 years of age or older, have a mental illness or substance abuse disorder, and have or are at risk for having a comorbid primary condition and/or chronic disease.
